The goal of this course is to learn the importance of health behavior theory in changing behaviors while considering the use of these theories in practice. This term, you will learn about a variety of concepts and constructs that will allow you note the importance of health behavior theory use in health promotion. Your final project is the summative application of this learning. You will apply a health behavior theory, model or framework to specific health behavior change through the development of an original health promotion concept/ idea.

The goal of the final project paper is to use the identified “change target” and the corresponding behavior theory, model or framework to propose a health promotion program. This program will explore the application of the chosen theory, model or framework to the “change target” in a step-by-step process. The health promotion program must be designed using the foundational components of the chosen health behavior theory, model or framework. Additionally, evidence of the following is required:

· General description of the health behavior. This will include relevant background information, epidemiological characteristics, and biostatistics relating to the behavior. Use this information to justify the need for your health promotion program.

· Identify a specific population in which the behavior poses a specific threat. Use biostatistics to support your emphasis on this population. Also, discuss the problem the behavior poses for the public health sector. Hint: How does this health behavior impact health outcomes and create public health challenges?

· Explain the basic constructs and concepts of the chosen health behavior theory, model or framework and explain how it is applied to behavior change. Use the Theory at a Glance document for guidance in choosing a theory, model or framework.

· Review current literature to discuss at least three current or previously implemented health promotion programs addressing this specific “change target” (health behavior). Explain how the programs were applied and the successes and failures noted. Also, identify the behavior theory, model or framework used in these programs. Note: It is acceptable if your reviewed programs target a different population than the one chosen for your project.

· Explain the basic premise of your health promotion program. This will include explaining how your program will apply the chosen health behavior theory toward meaningful behavior change. Specifically, identify program goals, strategies for addressing the problem meaningfully, activities to support those strategies. Refer to the supplemental documents Theory at a Glance andIntroduction to Health Promotion Program Planning (pgs. 26-48 only)for ideas and guidance. Be creative—don’t forget to give your program a name!

This written portion of your project will be presented in a 7 page paper and 7 Microsoft PowerPoint document summarizing the most important points from your paper (excluding title and reference pages). Use a minimum of 8 scholarly sources (references) and adhere to APA style (6th Edition) formatting for citation and paper structure.
